REGISTRATION
Registration is required for the Workshop. Please register via the ONLINE FORM no later than December 20, 2004.

FULL REGISTRATION INCLUDES:
Program, Badge, Access to All Sessions, All Meals, and Poster Session with Reception.
ONE DAY REGISTRATION INCLUDES:
Program, Badge, Access to All Sessions, Lunch, and Dinner.
PARTIAL DAY REGISTRATION INCLUDES:
Program, Badge, Lunch.
POSTER SESSION/RECEPTION INCLUDES:
Access to the Poster Session with Reception.
NO COST REGISTRATION INCLUDES:
Program (while they last) and Session Badge. There will be no fee for those planning to attend scientific sessions only; however, we would appreciate pre-registration by December 20, 2004 to facilitate Workshop coordination and to assure that you receive a pre-printed name badge.
CANCELLATION POLICY:A refund (less an $8.00 processing fee) will be given to any registrant who submits a cancellation notice by 5:00 p.m. CST on January 5, 2005. However, we will be unable to refund any fees for cancellations received after this date and time. All refunds will be processed after the conclusion of the Workshop.
