REGISTRATION FEES

Professional Registration $200.00
Accompanying Person $100.00

PROFESSIONAL REGISTRATION INCLUDES:

Program, Abstract Book, Badge, Access to All Sessions, All Meals and Breaks, Dinner Banquet and Transportation, and Welcoming Reception.

ACCOMPANYING PERSON REGISTRATION:

Welcoming Reception, Dinner Banquet, and Transportation to Dinner Banquet.

CANCELLATION POLICY:

A refund (less an $8.00 processing fee) will be given to any registrant who submits a cancellation notice by 5:00 p.m. CDT on April 21, 2003. However, we will be unable to refund any fees for cancellations after this date and time. All refunds will be processed after the conclusion of the Workshop.
