REGISTRATION

Registration is required for this Workshop. You may register via the online form at this address from February 16 through May 2, 2005.

Since a Workshop session will be held at the Brookhave National Laboratory, please be aware that all foreign nationals visiting the Laboratory must register via the Internet on the BNL Guest Information System (GIS) by March 15, 2005. Workshop participants should choose the Medical Department as the "Department/Division to be Assigned" and Marcelo Vazquez as the "Host." Foreign visitors should also be prepared to show a passport; citizens of the United States should bring photo identification.

Professional registration includes:
Program, Abstract Book, Badge, Access to All Sessions, All Meals and Breaks, Transportation to BNL Session, and Poster Receptions.

Student registration includes:
Program, Abstract Book, Badge, Access to All Sessions, All Meals and Breaks, Transportation to BNL Session, and Poster Receptions.

Accompanying person registration includes:
Welcoming Dinner, Poster Receptions, and Dinner Banquet.

No cost registration includes: Program (while they last) and session badge. A limited number of spaces are available (at no charge) for those planning to attend sessions only. However, we would appreciate pre-registration by May 2, 2005 to assure that you receive a pre-printed name badge and to facilitate Workshop coordination.

CANCELLATION POLICY
A refund (less an $8.00 processing fee) will be given to any registrant who submits a cancellation notice by 5:00 p.m. CDT May 7, 2005. However, we will be unable to refund any fees for cancellations received after this date and time. All refunds will be processed after the conclusion of the Workshop.
